The Housing Services Coordinator is responsible for reviewing, approving, and supporting requests for
housing-related services for Community Supports services offered by the health plan. This role ensures that housing related services are appropriate, align with program criteria, and support members stability and health outcomes. The position works closely with departmental clinical teams, care management teams, external housing providers, counties and community-based organizations to support access to safe, sustainable housing services and supports for members.
Education and Experience | Bachelor’s degree in Social Work, Public Health, Human Service or related

Special Skills, Licenses and Certifications | Familiarity with housing systems, landlord engagement, and/or homeless
|
Performance Based Competencies | Strong analytical skills and attention to detail for reviewing documentation and determining eligibility. Excellent communication and collaboration skills, both written and verbal. Ability to navigate electronic health records, care management and authorization systems.
