Overview

The Housing Services Coordinator is responsible for reviewing, approving, and supporting requests for
housing-related services for Community Supports services offered by the health plan. This role ensures that housing related services are appropriate, align with program criteria, and support members stability and health outcomes. The position works closely with departmental clinical teams, care management teams, external housing providers, counties and community-based organizations to support access to safe, sustainable housing services and supports for members.

Responsibilities

  • Reviews and processes referrals for housing-related services (e.g. Housing Transition Navigation
    Services, Housing Deposits, Housing Tenancy and Sustaining Services, Transitional Rent, etc.) in
    accordance with DHCS guidance and plan policies.
  • Evaluates documentation (e.g. housing plans, household goods, tenancy barriers, etc.) for completeness,
    eligibility, and compliance.
  • Makes and ensures timely authorizations or determinations on service requests within plan policies.
  • Collaborate with clinical and non-clinical staff to educate, support, and assist with members with
    housing related needs.
  • Serves as point of contact for members, care managers, and providers requesting housing service
    processes and benefit requirements.
  • Supports providers and case managers with technical assistance on service documentation, billing and/or
    reporting.
  • Maintains accurate documentation in case management and authorization systems.
  • Monitors service utilization, identify trend and flag issues or gaps in service access.
  • Collaborates with internal departments (e.g. Utilization Management, Case Management, Compliance,
    Provider Relations, etc.) to ensure consistency and quality of housing-related services.
  • Participates in audits, quality improvement initiatives, and reporting for housing-related services.
  • Other duties as assigned.

Qualifications

Education and Experience

Bachelor’s degree in Social Work, Public Health, Human Service or related
field preferred with 1-2 years of experience. In lieu of degree, 2-4 years of
experience in one or more of the following: Housing navigation or
supportive housing programs, case management or care coordination,
utilization management or service authorization experience in a health plan
setting, community-based behavioral health or social services.

 

Special Skills, Licenses and Certifications

Familiarity with housing systems, landlord engagement, and/or homeless
programs. Knowledge of Medicaid and/or Medicare benefits, CalAIM,
California’s Continuum of Care (CoC) system, Coordinated Entry,
Homeless Management Information System (HMIS) and/or social
determinants of health programs strongly preferred. Experience working
with populations experiencing homelessness, behavioral health needs or
complex social conditions.

 

Performance Based Competencies

 

Strong analytical skills and attention to detail for reviewing documentation
and determining eligibility. Excellent communication and collaboration
skills, both written and verbal. Ability to navigate electronic health records,
care management and authorization systems.

 

 

Work Environment And Physical Demands

Daily use of telephone and computer for most of the day. Cubicle
workstation. Ability to use a keyboard. Must be able to lift, move, or carry
objects of varying size, weighing up to 10lbs..
