Overview

To be responsible for managing the community engagement with our specific communities that
are experiencing health disparities in 24 counties and improving key disparities found with CAHPS
score. Establishes connections with various community-based organizations and translates their
needs to tangible actions that the health plan can act upon. Advises on Partnership’s community
engagement across multiple departments. Leads engagement with network providers,
subcontractors, downstream subcontractors to implement focus groups or individual 1:1 meetings,
across the network, to directly gather direct feedback from members. The position will lead a team
of specific community health needs liaisons that are specific to health disparities in key counties.

Responsibilities

▪ Identifies, establishes and manages partnerships with external organizations
specifically to advance Partnership’s health equity and social determinants of
health efforts.
▪ Assists with disseminating key information to specific community-related
committees/organizations and gathering key intelligence from key stakeholders.
▪ Identifies key external community events and provides guidance on how to
optimize engagement.
▪ Attends key external community events in combination with cultural and linguistic
liaisons and community health needs liaisons.
▪ Manages the Cultural and Linguistic-related deliverables from NCQA
accreditations and Quality Measure performance improvement activities.
▪ Manages and presents community-based organization or community member input
to various internal/external committees.
▪ Participates and works with other departments on cross-departmental initiatives that
require QIHEC input/participation.
▪ Tracks community engagement Equity budget spending, coordinates with director
and finance department as needed for projects, reimbursements and questions.
▪ Supports HEDIS performance improvement by conducting qualitative data analysis,
designing and implementing goals and interventions, and measuring results to evaluate
intervention and outcome.
▪ Will have oversight of producing annual reports regarding CAHPS related activities and
creates community needs assessment per disparity
▪ Other duties as assigned.

Qualifications

 

Education and Experience

Bachelor’s degree; At least a minimum of 4 to 6 years of experience
working directly in Health Equity or community engagement with a
proven track record of organizing various focus groups or other
community forums. Experience in leading and managing staff
preferred.

 

Special Skills, Licenses and Certifications

Strong understanding of community engagement Principles and
Concepts, theories, and best practices; Excellent compliance
management skills; Excellent verbal and written communication skills
to effectively convey community concepts and engage with various
stakeholders; Ability to assess the effectiveness of training programs,
gather feedback, and use data for continuous improvement. Valid
California driver’s license and proof of current automobile insurance
compliant with Partnership policy are required to operate a vehicle and
travel for company business.

 

Performance Based Competencies

Ability to handle multiple tasks simultaneously and remain cognizant
of the daily project management queue and the need to be compliant
with timeliness.

 

Work Environment And Physical Demands

More than 80% of work time is spent in front of a computer monitor.
Must be able to work in a high pace and high intensity environment.
