This position is responsible for working directly with local First Five Commissions and agencies
and providers who work directly with children, in Partnership HealthPlan of California
(Partnership) assigned counties. The goal of this position is to ensure youth obtain the health
services they are entitled to and that services are closely coordinated with other services. This
position will assist in the design, implementation, and/or expansion of strategic programs and
departmental initiatives in relation to this population and regulatory deliverables. Develops and
delivers program goals, measures, and reports. This position will be responsible for a range of
systems to strengthen coordination and supports for children and youth and will facilitate
communication with external stakeholders including: First Five Commissions, network
providers, county staff, birth families and other stakeholders as appropriate.
▪ Serves as the lead point of contact on behalf of Partnership with external First Five
serving stakeholders.
▪ Oversees and develops a system for care coordination for this designated population on
behalf of the plan ensuring care coordination staff who are capable serving as the primary
point of contact responsible for the child’s care coordination.
▪ Serves as the Partnership lead for oversight of the Memorandum of Understanding
(MOU) between Partnership and First Five Commissions in the Plan’s 24 counties,
ensuring Partnership compliance with the requirements of the MOU.
▪ Ensures there is a process for training and education around the requirements of the
MOU.
▪ Conducts focus groups and surveys with external stakeholders to ensure systems are in
place to be responsive to the needs of this special membership population.
▪ When indicated, establishes systems to ensure connections with county mental health
plans for the delivery of specialty mental health services on behalf of this specific
population.
▪ Serves as Partnership’s First Five liaison by overseeing identified liaison designees to
relevant local Commissions, agencies and ECM providers who work directly with
children and youth.
▪ Supports the development of an ECM network specific to meet the needs of this child and
youth population.
▪ Develops a process to ensure appropriate participation in local coordination meetings and
responds to the health needs of this population.
▪ Provides formal presentations to both external and internal stakeholders on matters
relevant to this population and regulatory requirements pertaining to it.
▪ Follows DHCS-issued standards and expectations as set forth in APLs or other similar
instructions.
▪ Coordinates with ECM Providers providing services to children and youth, provides
technical assistance, as needed.
▪ Serves as a point of escalation for care managers if they face operational obstacles when
working with community partners.
▪ Ensures covered services are closely coordinated with other services, including social
services and specialty mental health care services, when appropriate.
▪ Travels and works with local agencies and ECM providers in Partnership counties to
engage and create social and professional relationships.
▪ Responsible for educating and coordinating providers and teams on appropriate referrals.
▪ Responsible for supporting resolution of payment for services provided.
▪ Maintains a database of key information about local child serving agencies and ECM
providers in our service area.
▪ Creates and delivers program information and reports to both internal and external
stakeholders.
▪ Develops and implements strategies to engage stakeholders.
▪ Leads program evaluation and continuous improvement.
▪ Guides program staff in successful implementation of program activities.
▪ Develops and executes program communication plans and updates.
▪ Reviews program data accuracy, completeness, and required submissions.
▪ Schedules and facilitates program meetings.
▪ Identifies and resolves program issues and concerns with both internal and external
stakeholders to meet the needs of all constituents and to ensure risk is mitigated in a
timely manner.
▪ Recommends solutions to senior leadership for final decisions as necessary.
▪ Serves as the Plan leader and advocates on behalf of the child and youth around their
health needs.

$136,296.78 - $177,185.82
