Provides oversight of Care Coordination programs and initiatives implemented by the Care Coordination Team designed to improve the health of PHC members and to provide excellent customer service to Members and Providers.
SECONDARY DUTIES AND RESPONSIBILITIES
Education and Experience | Registered Nurse with a Bachelor’s degree required. Master's preferred. At least five (5) years of experience in a leadership/management role required. Experience in a managed care enviornment preferred.

Special Skills, Licenses and Certifications | Current California Registered Nurse license. Must have excellent verbal and written skills. Valid California driver’s license and proof of current automobile insurance compliant with PHC policy are required to operate a vehicle and travel for company business.

Performance Based Competencies | Strong knowledge of computers, quality improvement processes, and data collection. Demonstrated effective leadership and analytical skills. Excellent interpersonal skills.

Work Environment And Physical Demands | Occasional travel required. Ability to use a computer keyboard. Ability to prioritize workload and function effectively with frequent interruptions and direction from multiple team members. More than 50% of work time is spent in front of a computer monitor. Must be able to lift, move, or carry objects of varying size, weighing up to 10 lbs.
