Overview

In collaboration with departmental and organizational leadership, supports the organization’s Population Needs Assessment activities through active and meaningful engagement in identified community workgroup and initiatives. On behalf of the health plan identifies and supports, key strategic activities and interventions that support alignment of collective agency efforts that promote and support efforts to encourage member health outcomes.

Responsibilities

  • Works collaboratively with leaders and community partners in the development of the
    health plan’s Population Needs Assessment (PNA) through active participation in the
    Local Health Department’s Community Health Assessment (CHA) and/or Community
    Health Improvement Plan (CHIP) processes.
  • Helps to develop, track, and monitor the health plan’s shared goals with the Local
    Health Department’s Community Health Improvement Plan (CHIP).
  • Conducts outreach and engagement to build relationships with stakeholders including,
    but not limited to: Local Health Departments, hospitals, providers, community-based
    organizations (CBOs), education agencies, grass roots organizations, and other external
    partner agencies
  • Fosters positive working relationships with providers, members, and/or community
    stakeholders to promote organizational objectives and mission.
  • Conducts research to gather information on identified community gaps, needs and/or
    available resources.
  • Participates in internal and external workgroups that address health equity and/or health
    disparities
  • Makes recommendations for activities that promote alignment across multi-sector
    organizations and agencies
  • Shares and presents qualitative and quantitative data with a variety of stakeholders
    including, but not limited to: Partnership leadership, Local Health Departments,
    hospitals, community stakeholders, and/or members
  • Provides community resources and education at community collaborative meetings and
    community events as assigned, including education on Partnership benefits, services,
    updates, wellness programs, and current regulations as applicable in support of improving
    the health and wellbeing of PHC members.
  • Maintains an active working knowledge of regulatory and/or accreditation requirements
    related to DHCS and NCQA Population Needs Assessment activities and serves as a
    subject matter expert.
  • Assists the department in completing the Population Needs Assessment process for
    DHCS auditing and/or NCQA survey processes
  • As assigned, supports the health plan’s requirements and activities under applicable
    memorandum of understanding (MOU) with provider/community partners.
  • Works with organization’s health educators to identify appropriate member educational
    materials to share with community partners, providers, and members.
  • Performs other duties as required.

Qualifications

Education and Experience

Bachelor’s Degree in Public Health, Community Health or related
field. Master’s degree preferred. At least two (2) years of experience in
public health. Experience in a managed care and/or working with
Medi-Cal population preferred.

 

 

Special Skills, Licenses and Certifications

Must have excellent verbal and written skills. Experience in working
with diverse and/or community groups. Experience public speaking
and/or presenting materials to a variety of audiences. Valid California
driver’s license and proof of current automobile insurance compliant
with Partnership policies are required to operate a vehicle and travel on
company business.

 

 

Performance Based Competencies

Excellent written, verbal, and interpersonal communication skills
including the ability to express oneself clearly and concisely when
providing service to internal departments, members, providers and
outside entities over the telephone, in person or in writing. Ability to
understand, analyze, and operationalize regulatory requirements.
Ability to analyze and interpret data. Proficiency in community based
programs and/or county public health services.

 

Work Environment And Physical Demands

Occasional travel required up to 25%. Ability to use a computer
keyboard. Ability to prioritize workload and function effectively with
frequent interruptions and direction from multiple team members.
More than 50% of work time is spent in front of a computer monitor.
Must be able to lift, move, or carry objects of varying size, weighing
up to 10 lbs
